全一卷。唐代菩提流志譯於景龍三年（709）。為密教用以消災祈福之經典之一。收於大正藏第二十冊。本經共有十品，內容敘述佛應觀自在菩薩之請而宣說根本陀羅尼、大心陀羅尼、小心陀羅尼，並說誦持此等陀羅尼之種種功德，及其受持方法。
　本經之異譯本有三：(一)實叉難陀所譯之觀世音菩薩祕密藏如意輪陀羅尼神咒經，(二)寶思惟所譯之觀世音菩薩如意摩尼陀羅尼經，(三)義淨所譯之觀自在菩薩如意心陀羅尼咒經。此三種異本與本經皆譯於唐代，其中以菩提流志所譯者為最完備。〔開元釋教錄卷九、貞元新定釋教目錄卷十四〕
